Report a Complaint or Schedule a File Review

Report a Complaint - the following information needs to be provided to the appropriate Western Region DEQ office:

• Date and time incident observed
• Source location (street address, city, county, zip code, and telephone number)
• Description of incident
• The person reporting incident

Schedule a File Review - File reviews are done by appointment only.  You must contact each office individually to review files.  We can try to accommodate multiple reviews but due to the high volume of requests, please contact a Western Region DEQ office.

To get site information before calling to set-up a file review, you can visit our Facility Profiler to get the file number; or you can obtain Site Summary reports for Environmental Cleanup Site Information (ECSI) and Water Quality Limited Streams and Permitted Facilities.
