Discharge Monitoring Report (DMR) Forms for NPDES and WPCF General Permits

If you hold a General Permit and it appears in the list below, you need to submit regular discharge monitoring reports to DEQ on forms provided by the Department. To get a copy of the DMR report form for your permit, select the General Permit type, enter your File Number1 and click on Submit.

Select the permit type and enter a file number
Select General Permit Type
Enter File Number1 or leave blank

1The File Number appears under "Issued To:" on the face page of your General permit.
This feature is not available for 1200-COLS and 1200-Z.
