The Form

The form for requesting information from the LUST Cleanup Site Database contains nine fields [1-LUST Number, 2-County, 3-Year, 4-Region, 5-Site Name, 6-Address, 7-City, 8-Zip Code, and 9-Underground Storage Tank (UST) Facility Number], four checkboxes (10-DEQ Cleanup File Open, 11-Unregulated Heating Oil Tank, 12-Unregulated Tank, and 13-Regulated Tank), and two buttons (14-Lookup, and 15-Clear). Two of the fields (County and Region) have drop-down lists that provide you with the allowable choices.

Performing a Search

When you want to search for data, you only need to enter information into as many fields and check as many boxes as are necessary to produce the dataset that you want. Also, in most of the fields you can provide partial information and the search will produce all entries that include that part within the overall field.

After entering information into the fields, click on the "Lookup" button to perform the search. Depending on how many entries are found, this may take a while. You will then see a form which lists all of the sites that meet your search requirements. The results are listed in a table which includes the Log Number, Site Name, Address, City, Zip, and County.

You can sort this table by clicking on any of the six heading names. If you want them sorted by zip code, for example, simply click on the word "Zip" at the top of that column. To view the information for a particular site, click on the specific log number for that site in the first column of data. This will generate a site-specific form listing information that is contained in the LUST database.

NOTE: Depending on the priority of the site, when the latest report was submitted, if a project manager is assigned to that site, and other factors, that form may not be completely up-to-date. If you need to know all of the latest information about a project, you should contact the appropriate Regional Office and make an appointment to view the project file.

To begin a new search, you can click on the "Clear" button to delete all of the entries from the form, or you can change one or more entries by deleting and typing them individually.

Several examples of LUST database searches are listed below:

• You're working on a specific project and want to check the status in the LUST database. Even though you have all of the information -- LUST number, site name, address, etc. -- you only need to enter the LUST Number, then click on the "Lookup" button to produce a list that should only contain one entry. Clicking on the log number will produce the report for that site.
• You want a list of all reported releases for Marion County in 1999, sorted by city. Select "Marion" from the County drop-down list, type 1999 into the field marked "Year," and then click on the "Lookup" button. When the list of sites appears, click on "City" to sort the list by city name.
• You want a list of all reported releases for unregulated heating oil tanks in Lane County in 1999, sorted by zip code. Select "Lane" from the County drop-down list, type 1999 into the field marked "Year," click on the box by "Unregulated Heating Oil Tank" (a check mark should appear in that box), and then click on the "Lookup" button. When the list of sites appears, click on "Zip" to sort the list by zip code.
• You know there was a release from a site on Cornell Road in Washington County, but you don't know the address and are not sure what year the release may have occurred. Select "Washington" from the County drop-down list, type Cornell Road in the field marked "Address" and click on the "Lookup" button.

Because many fields are available for your use, there may be several ways to perform a given search. For example, the initial search described under the second bullet above could have been accomplished by typing 24 (the number for Marion County) into the first part of the LUST number field and 99 (the year) into the second part. By leaving the third part empty, all releases in Marion County in 1999 will be listed.

Fields, Checkboxes and Buttons

1. LUST Number: This is an eight-digit site identification number that looks something like this: 15-99-0067. If your facility has had a release, letters from the Department would contain this LUST log number.

The first two digits are a code for the county in which the release occurred. The numbers of each of Oregon's 36 counties can be found on the County List on the UST Program Regional Office page. The second two digits are for the year in which the release was reported; 99 represents 1999, 01 represents 2001, etc. The final four digits are the number of the release in the listed county for that given year. So, 15-99-0067 stands for the 67th reported release in 1999 for county 15 (Jackson County).

When you use the LUST number field you may enter all 8 digits if you know them, or you may enter only one or two parts of the number, such as just the county number, or just the county and year. You do not need to enter any hyphens, just put the digits in the appropriate boxes. If you only enter part of the 4-digit release number, the search will lookup all release numbers that start with the digits you entered.

For example, if you entered "15" and "99" and "00" into the form, you would get a list of all reported releases in 1999 for county 15 that were assigned release numbers beginning with "00". So, 15-99-0067 would be one of the sites contained on this list.

2. County: To enter a county name, click on the arrow to reveal the list of names. Then scroll down and click on the county of interest.
3. Year: Type a 4-digit year in the box, such as 1999, or 2001.
4. Region: To select a region, click on the arrow to reveal the list of names. Then scroll down and click on the region of interest.
5. Site Name: This is the name assigned to the project by the DEQ, such as John Smith Texaco, or River City Auto, etc. If you don't know the complete name you can enter a partial name to get a list of sites containing that word or name. For example, you could search for all site names that include the name "Chevron" by typing that in the "Site Name" field and clicking on search.

NOTE: In the site printout all heating oil tank sites will simply be listed as "Heating Oil Tank" in the "Site Name" column. This is done in order to maintain privacy and prevent the release of the homeowner's name.

6. Address: This is the mailing address of the site. If you only know the street name you can enter that in the "Address" field and get a list of all reported releases on that street. Or, you can enter just an address number without the street name (such as 5600) to narrow your search to one or only a few sites.
7. City: This is the city name contained in the mailing address. Note that there are cases where this may not be within the city limits. For example, many businesses in unincorporated Washington County have Portland mailing addresses, even though they are NOT within the Portland city limits. The same is true for some locations in Clackamas County.
8. Zip Code: This is the zip code associated with the mailing address for the site.
9. UST Facility Number: This is the UST Facility Identification Number that the Department assigns to each facility with regulated underground storage tanks. Releases at unregulated facilities, such as residential heating oil tanks, will not have a facility number.

NOTE: If you entered the facility ID for a site and the search did not return any results, you may want to search by address since facility IDs have not always been entered into the database.

10. DEQ Cleanup File Open: Check this box if you only want to see the facilities that have not yet been "closed." An open site is one that has not yet met the requirements of the UST Cleanup Rules to receive a "No Further Action" letter from the Department, or has not yet been reviewed by the DEQ for closure.
11. Unregulated Heating Oil Tank (HOT): Check this box if you only want to see releases reported from unregulated heating oil tanks (HOTs), such as residential HOTs.
12. Unregulated Tank (Non HOT): Check this box if you only want to see releases reported from unregulated tanks OTHER than HOTs, for example, farm fuel tanks containing less than 1100 gallons.
13. Regulated Tank (UST Facility): Check this box if you only want to see releases reported from regulated UST facilities.
14. Lookup Button: Click on this button to perform a data search after you have entered data into appropriate fields and checked any necessary checkboxes.
15. Clear Button: Click on this button to delete all of your entries from the form.
