RCRA Site Identification NumbersA RCRA site identification number is required for certain facilities managing wastes under the Resource Conservation and Recovery Act (RCRA). The site identification number is required for facilities notifying the DEQ of their hazardous waste, universal waste or used oil activities. The Site Identification Form (SI Form) available below is used by facilities that are required to notify the Oregon DEQ that they are involved in these waste activities. Facilities that have already notified DEQ of their waste management activities may also use this form to update site information. For example, an updated form should be submitted when there is a change in ownership, when a business closes, or whenever you would like to update previously submitted site information.
