Report a Complaint or Schedule a File Review

Report a Complaint - the following information needs to be provided to the appropriate Eastern Region DEQ office:

• Date and Time Incident Observed
• Source Location (street address, city, county, zip code, and telephone number)
• Description of Incident
• Name and Contact information of Person Reporting Incident

Schedule a File Review - File reviews are done by appointment only.  You must contact each office individually to review files.  We can try to accommodate multiple reviews but due to the high volume of requests, please contact an Eastern Region DEQ office.
