Applications and reporting forms 

ACDPs are primarily used to regulate minor sources of air contaminant emissions, but are also required for any new major source or major modification at a major source. There are six types of ACDPs. A General ACDP differs from the other ACDPs in that they are issued for an entire source category, and qualifying facilities are subsequently assigned to the permit. The other types of ACDPs are issued for each individual facility.

  • Administrative forms: Notice of Intent to Construct (AQ104), Notice of Approved Construction Completion (AQ104C), Non-technical permit modifications (AQ103 and AQ105) and *Construction Approvals – Form AQ104.

    *For sources not required to obtain a permit and for notifying the Department of some types of changes at permitted sources
